Required Documents For Smart Card [Ex-Servicemen, New Card]

• Copy of Ex-serviceman identity card (Zilla Sainik board)
• Copy of discharge book or release order or PPO
• Copy of liquor permit – excise office (‘A’ Div or Old Custom House)
• NOC or Affidavit in case of delay beyond 6 months after retirement.
• Residential Proof. (Ration Card or Electricity Bill)
• Photographs (2 Single or 2 Joint)
• Death certificate for widow only.
• Cost of the smart card of Rs. 135/- each & smart card form fill up in black pen.

Note:
⇒ On receipt of SMS regarding collection of Smart Cards from Station HQs, a mobile number will be provided in the same SMS.
⇒ Forward a SMS from each registered mobile number against card to this number in the format.
⇒ A confirmation SMS will be forwarded to your registered mobile number regarding
activation of Smart Card.
